We Make it Easy as 1-2-3!

When it comes to choosing a medical provider or facility, it helps to see how your options work — and what it could cost you. That’s exactly what this page is designed to do. By following these steps, you can find the care that’s the best fit for your health needs and your wallet.

1. Understand Your Choices Before You Receive Care

We offer two medical plan options to fit different needs and budgets. Each plan covers preventive care at 100% and includes valuable resources like Telehealth and mental health support. Watch our videos to learn more about the PPO, the HDHP, and how an HSA can help you save money now and in the future. Then review the summaries below to decide which plan is right for you.

HRA

PPO PLAN

The PPO plan gives you predictable copays for office visits and more up-front coverage, but it also has a higher paycheck cost than the HDHP. This plan may be a better fit if you want to know exactly what you’ll pay when you see the doctor, and you don’t mind paying a little more each pay period for that predictability.

HDHP PLAN

The HDHP has lower premiums, which means you’ll keep more of your paycheck, but you’ll pay the full cost of care until you meet the deductible. After that, you’ll share costs with coinsurance. This plan pairs with a Health Savings Account (HSA), which lets you save pre-tax money to help cover those out-of-pocket costs. It can be a smart choice if you don’t expect many medical expenses or want the ability to build long-term savings for future healthcare needs.

HOW TO USE YOUR HSA

An HSA is your personal account for healthcare spending and saving. You put money in tax-free, it grows tax-free, and you can take it out tax-free when you use it for qualified expenses. Unlike an FSA, your HSA dollars roll over from year to year and the account stays with you if you change jobs or retire. Over time, it can become both a safety net for unexpected bills and a long-term savings tool for future healthcare costs.

2. Medical Plan Comparison

Understanding the basics of each plan is the first step in making the right choice. The chart below shows a side-by-side comparison of your medical plan options, highlighting key costs like deductibles, copays, and coinsurance. Use this chart to get a feel for how each plan handles different types of care — from routine visits to major health events. As you review, think about how you typically use healthcare and which plan might work best for your needs and budget.

3. See How the Plans Work in Real Life

It can be hard to picture how a health plan actually works until you use it — so we’ve put together a few examples to show you. Each one highlights a different type of person with unique healthcare needs, from someone who barely visits the doctor to someone managing a chronic condition. For each example, you’ll see how much care they needed, what the plans covered, and what they paid out of pocket. Click below to meet these individuals and find the situation that looks most like yours — it’s a helpful way to see how your own costs could add up under each plan.

Meet Jake

See how Jake keeps costs low while staying covered for the unexpected — all with Employee Only coverage.

Health Snapshot

Jake is a 28-year-old single man who elects Employee Only coverage. He’s healthy and generally only needs annual preventive care and the occasional sick visit. With limited savings, Jake wants a plan that keeps out-of-pocket costs low while still giving him access to high-quality care if something unexpected happens.

Jake's Care Journey

Jake makes his health a priority with preventive care and an annual check-up. When minor issues pop up, like a sinus infection during allergy season, he opts for the convenience of a virtual visit and a low-cost generic prescription.

This year, his care was simple — one preventive exam, one virtual visit, and a prescription refill. With such minimal use, Jake benefited most from the HDHP. The plan’s low premiums and Sample Company’s HSA contribution helped cover his small expenses while keeping his overall costs down. Most importantly, Jake feels confident knowing he’s protected if something unexpected happens — all without overpaying for coverage he rarely needs.

A Win for Jake — and the Company

Choosing the HDHP lets Jake keep more of his paycheck while still having reliable protection for big, unexpected expenses. The lower premiums and Sample Company’s HSA contribution make it easy for him to cover the occasional visit without dipping into savings.

For Sample Company, this is a win because employees like Jake are making cost-conscious choices that align with how they actually use healthcare. Encouraging enrollment in the HDHP helps the company control overall plan costs, reduce premium growth, and reinvest savings into valuable benefits for the entire workforce. It also empowers employees with tools — like HSA contributions and preventive care — so they feel supported while managing their healthcare dollars wisely.

“I mostly just need preventive care and the occasional sick visit. With the right plan, I can keep costs low and still be covered if something unexpected happens.”

—Jake L

Meet The Martins

See how Claire and Ben keep their busy family on track with a plan that helps them manage care for everyone — while keeping more money in their pocket each month.

Health Snapshot

Claire is a Sr. Accounts Payable Specialist at Sample Company. She and her husband, Ben, have two active kids who need regular pediatric care — including well-child visits and vaccinations. Sometimes, life throws in surprises, like when their daughter broke her arm, requiring an ER visit, imaging, and follow-up care with an orthopedic specialist. Claire also prioritizes her well-being by scheduling regular therapy sessions to manage stress and stay balanced.

Because they participate in the LiveWell program to maximize their benefits, Claire and Ben chose the HDHP, paired with Sample Company’s $1,200 HSA contribution. This combination helps them control monthly costs and prepare for unexpected medical expenses while keeping more money in their pocket for family priorities.

The Martin's Care Journey

With two active kids, the Martins make regular use of pediatric care, including well-child visits and vaccinations. They occasionally need urgent care or specialist visits—like when their daughter broke her arm falling off the monkey bars, leading to an ER visit, imaging, and follow-up care with an orthopedic specialist.

In addition to caring for her kids, Laura prioritizes her own well-being by seeing a therapist regularly for mental health support. For this family, a plan that combines lower premiums with DFW’s generous HSA contribution makes sense. It helps them manage both routine care and unexpected medical costs without breaking their budget.

A Win for the Martins — and Sample Company

By choosing a plan that balances affordability, flexibility, and preventive care, the Martins keep their family healthy while avoiding financial strain. The HSA contribution gives them an extra cushion for unplanned expenses, and their commitment to in-network care helps control claim costs—supporting long-term savings for the company and all employees.

When families like the Martins use their benefits strategically, everyone wins—better access to care and a sustainable benefits program for the future.

“Between routine check-ups, urgent care for the kids, and my own mental health appointments, we need a plan that works for our whole family — and gives us security when the unexpected happens.”

—Claire M

See how Susan and Dave manage chronic conditions and plan for surgery with a plan that provides full coverage and peace of mind.

Health Snapshot

Susan and Dave are in their early 60s and elect Employee + Spouse coverage. With both managing chronic conditions — and Susan preparing for knee surgery later this year — they need a plan that provides comprehensive coverage with predictable out-of-pocket costs and access to trusted providers.

The Martin's Care Journey

The Parkers use their healthcare benefits extensively. Dave manages diabetes, including routine visits to his endocrinologist and ongoing insulin prescriptions. Susan is currently in physical therapy to prepare for knee surgery and regularly sees a therapist to support her mental health during recovery.

They rely on their plan to cover a wide range of services, from specialist care and prescriptions to surgery and outpatient rehab — and want the flexibility to see out-of-network providers if needed.

A Win for the Parkers — and Sample Company

With high medical usage, Susan and Dave depend on a plan that provides extensive coverage without creating financial stress. By making smart use of in-network providers, they manage their chronic conditions and major care needs while helping reduce overall claim costs.

When employees with complex medical needs have access to coordinated, cost-effective care, it supports better health outcomes — and helps the company maintain a strong, sustainable benefits offering for all.

“We both have ongoing medical needs, so having a plan that covers everything from prescriptions to surgery — and helps us manage costs — is essential.”
